How Do I Qualify for Prescription Assistance?

You may be eligible if you:

  • Don’t have prescription insurance coverage.
  • Have difficulty affording your medications.

If either applies to you, our Prescription Assistance team can determine your next steps and provide you with options.

What Should I Bring to My Appointment?

When you meet with a representative of our Prescription Assistance team, please have:

  • Proof of income. That could be your federal tax return or documentation of your monthly household income (salary, pension, Social Security, etc.).
  • Current medication bottles or prescriptions for all the medicines you take.

We’ll keep all the information you share confidential and use it only to match you with the appropriate assistance program. At the conclusion of your appointment, we’ll provide you with paperwork. Take it to your primary healthcare provider’s office for a signature and submission.

How Will I Get My Medication?

If approved for assistance, you’ll receive step-by-step instructions on where to get your medications: through a pharmacy, by mail or at your doctor’s office. We’re here to make the process simple and supportive, so you won’t have to navigate it alone.
