Receiving Copies of Your Medical Records
Accessing your health information is simple and secure. Through Health Information Management you can request test results, authorize record sharing and choose how you want to receive your medical information, all while protecting your privacy.
How to Receive a Copy of Your Medical Records
You have a few options for receiving your medical records. Fisher-Titus recommends starting with the Patient Portal for the fastest and easiest access.
Patient Portal (recommended and no authorization required)
Log in to the Fisher-Titus Patient Portal to view and download your medical records securely online.
Fill Out a Request Form
If you need records not available through the portal, complete our Medical Records Request Form.
- You can use Adobe Fill & Sign to submit it electronically, or print and return it in person or by email to recordsrequest@ftmc.com.
- A valid photo ID may be required before records can be released.
- Once received, we’ll release your records to you or the provider/organization you’ve authorized.

If you cannot use the options above, call 419-660-2702, Monday–Friday, 7:30 a.m. to 4:00 p.m. Please note: test results cannot be shared over the phone, even with your verbal permission, to protect your privacy.
