Fisher-Titus Updates Procedures for Medical Records and Financial Counseling During COVID-19

In order to protect patients and staff to any unnecessary COVID-19 exposure, Fisher-Titus has made adjustments to requesting medical records and financial counseling.

Walk-in requests for medical records will no longer be accepted as the Health Information Management window has been closed. To request a copy of your medical record, please call the Health Information Management direct line at 419-660-2702 during the hours of 7am-4:30pm, Monday through Friday. If leaving a message, someone will contact you within two working day of your message. Options for requesting and receiving your medical record will then be reviewed with you.

In addition, Fisher-Titus has closed the Cashier window and is asking people to call, mail, or visit online to make payments. Please do not hesitate to call the number on your statement with any questions.

For those who must drop off a payment or papers, Fisher-Titus Financial Counselors will not be able to have a conversation with you in the office; however, they can have a phone conversation to answer any questions and provide guidance. This will allow them to continue to serve you while complying with the 6’ social distancing guidance. Fisher-Titus Financial Counselors can be reached at 419-660-2678 or 800-589-3862 from the hours of 8 a.m.-5 p.m., Monday through Friday.
